CSI400  400W 100V 5A special metal halide bulb by General Electric.

OK, so this is one very special bulb, as used in laboratories and high end scanning equipment.

You don't really want to be looking at this bulb unless as a replacement for existing unit, and there is no option !!!

These have a very short lifespan, and look dead even before you take it out of the box, no filament, just a couple of cattle prods in a high vacuum squat glass capsule, which also contains a blob of liquid metal.
